Public Records Request Form

Seal In accordance with the Louisiana Public Records law, La. Rev. Stat. 44:1, et seq., St. John the Baptist strives to promptly release public records upon request. Because staff must research and retrieve the specified records, sometimes with third party assistance, some record requests require additional time. If at all possible, the requested records will be provided as quickly as possible or a notification of the status of the request will be provided within a 5 day period.

Fees: Copies will not be made until written confirmation of the order is received and all applicable fees are paid in full. If a written confirmation or payment is not received within 10 business days after notification of the estimated fees, the requestor may be required to initiate a new request.

The requestor also has the option of reviewing records onsite at an agreed upon time and location in the presence of a parish employee or designee.

Uniform Fee Schedule:The following Uniform Fee Schedule must be used when furnishing copies of public records requested by the public.

Copy up to 8 ½ by 14 inches paper $.25 per one-sided copy

$.50 per two-sided copy
Copy of existing electronic files or an electronic copy
$.25 per one-sided paper copy

$.50 per two-sided paper copy
Copies printed or produced by outside sources at the request of the department Actual Cost
Surcharge for every 100 pages copied $10

 

Additional service fees may apply depending on the scope of the request.

All fees must be paid prior to the release of the records.
